If bacteria get into the tooth pulp, also called pulp, they can cause root inflammation. Root canal treatment becomes necessary to save the tooth.
The cause of inflammation of the nerve in the root canal is usually deep caries. However, bacteria can also enter the interior of the tooth if the tooth is broken off in an accident. In rare cases, a filling near the nerve can also cause similar inflammatory reactions of the nerve. If the inflammation remains untreated, the bacteria can spread further into the surrounding tissue via the root tip. As a symptom, the classic, painful "thick cheek" with pus accumulation may develop. A painless death of the tooth nerve is also possible. In this case, the local chronic inflammation can lead to stress of the whole body. X-ray examination is necessary for diagnosis.

In very rare cases, complications may arise due to complex anatomical shape of the root canal. In this case, parts of the root canal exist which could not be disinfected during endodontic treatment (root canal treatment). The result is a renewed inflammation of the root, which can lead to pain and swelling (e.g. abscess).
In such cases there are three options: 1. tooth extraction 2. root apex resection, 3. revision treatment.
In preparation for the treatment, the dentist can attach a small elastic cloth around the tooth. This so-called cofferdam keeps the tooth free of saliva as well as bacteria during the root canal treatment and facilitates the treatment. At the beginning of the treatment, the dentist usually opens the tooth under local anesthesia. He removes the infected tissue with very thin instruments. In addition, the dentist rinses the root canal with an antibacterial solution. At the same time, he uses X-ray images to check the success of the treatment. In an apicoectomy, the dentist cuts off about two to three millimeters of the root tip. This involves removing the bone above the affected root tip to access the inflamed tissue. The dentist then prepares and fills the root canal. As a rule, the operation is performed on an outpatient basis under local anesthesia.
During a revision treatment, the existing root canal filling is revised, and the root canals are prepared, disinfected and filled again. The aim of a revision treatment, as with a root canal treatment, is to disinfect and clean the root canal.
Harmless side effects of any root canal treatment may include bite sensitivity or moderate pain. Occasionally, antibiotics are also useful to combat an infection. The long-term prognosis of a root canal treatment is impaired if it is not possible to completely prepare particularly narrow or curved canals or canal sections. Other possible complications include perforation - that is, the instruments create an additional exit from the root canal system - or breakage of the hair-thin instruments. If the tooth is still unable to bear weight for a long time after the treatment has been completed, or if inflammation around the root is visible in the X-ray, further treatment will be necessary. To clarify this in detail, the dentist can consult an endodontic expert who specializes in treating the diseased interior of the tooth.
Depending on the cause of the failed treatment, further preparation may be necessary to clean and fill the last remnants of the microfine root canals. Another possible treatment step to save the tooth is surgical removal of the root tip or, in the case of a multi-rooted tooth, complete removal of a root.
As a basic rule, the more substance loss, the more fragile the tooth. As a rule, root-treated teeth are at risk of fracture. In exceptional cases, a post build-up or a single-surface filling is sufficient for long-term success. A ceramic crown could protect the tooth in the long term and improve its appearance.
In the past, root canal-treated teeth often turned dark because staining medications or fillings were used. Today, such substances are no longer used. Nevertheless, the color of a root canal-treated tooth may later become somewhat gray, for example. However, discoloration can only occur with natural teeth; this risk does not exist with artificial crowns. Discoloration can often be reversed by whitening inlays in the tooth (bleaching). Lasting color corrections are also possible with veneers or veneered crowns.
