If bacteria enter the pulp of the tooth, they can cause root inflammation. Root canal treatment becomes necessary to save the tooth.
The cause of inflammation of the nerve in the root canal is usually deep caries. However, bacteria can also enter the inside of the tooth if it has been broken off in an accident. In rare cases, a filling close to the nerve can also cause similar inflammatory reactions of the nerve. If the inflammation remains untreated, the bacteria can spread via the root tip into the surrounding tissue. The classic, painful “fat cheek” with an accumulation of pus can develop as a symptom. Painless death of the tooth nerve is also possible. The local chronic inflammation can lead to the whole body being affected. An X-ray examination is necessary for diagnosis.

No. It even relieves you of the toothache. The tooth is treated under local anesthesia so that you do not feel any pain during the treatment. Most patients feel relieved and liberated after root canal treatment.
In very rare cases, complications can occur due to the complex anatomical shape of the root canal. In this case, there are parts of the root canal that could not be disinfected during endodontic treatment (root canal treatment). The result is renewed inflammation of the root, which can lead to pain and swelling (e.g. abscess).
In such cases, there are three options: 1. tooth extraction, 2. apicoectomy, 3. revision treatment.
In preparation for the treatment, the dentist can place a small elastic cloth around the tooth. This so-called cofferdam keeps the tooth free of saliva and bacteria during the root canal treatment and makes the treatment easier. At the beginning of the treatment, the dentist usually opens the tooth under local anesthesia. He removes the infected tissue with very thin instruments. The dentist also rinses the root canal with an antibacterial solution. At the same time, he checks the success of the treatment using X-ray images. Electronic root canal length measurements are helpful during treatment. Several sessions may be necessary. In the meantime, the dentist treats the tooth with an anti-inflammatory or antibacterial insert and closes the tooth temporarily. It is important to remove as much bacteria as possible from the canal. The root canal is then given a bacteria-proof filling – usually made of gutta-percha, a rubber-like substance.

The aim of root canal treatment is to disinfect and clean the root canals. The more curvatures and undermining shapes the root canals have, the greater the challenge. With the help of the laser, we are able to clean even complex roots.
In order to clean the entire root canal, it is necessary to measure the root canal accurately. Electrical length measurement in combination with X-ray imaging is currently the best way to determine the length of the root canal.
During an apicoectomy, the dentist removes around two to three millimetres of the root tip. The bone above the affected root tip is removed in order to access the inflamed tissue. The dentist then prepares the root canal and fills it. The operation is usually performed on an outpatient basis under local anesthesia.
As part of a revision treatment, the existing root canal filling is revised and the root canals are prepared, disinfected and filled again. As with root canal treatment, the aim of revision treatment is to disinfect and clean the root canal.
Harmless side effects of any root canal treatment can be bite sensitivity or moderate pain. Occasionally, antibiotics are also useful to combat an infection. The long-term prognosis of root canal treatment is impaired if it is not possible to completely prepare particularly narrow or curved canals or canal sections. Other possible complications include perforations – i.e. the instruments create an additional exit from the root canal system – or breakage of the hair-thin instruments. If the tooth is still not able to bear weight a long time after the treatment has been completed or if inflammation around the root is visible on the x-ray, further treatment will be necessary. Depending on the cause of the failed treatment, further preparation may be necessary to clean and fill the last remnants of the microfine root canals. The basic rule is: the more substance loss, the more fragile the tooth is. As a rule, root canal-treated teeth are at risk of fracture. In exceptional cases, a post build-up or a single-surface filling is sufficient for long-term success. A ceramic crown could protect the tooth in the long term and improve its appearance.
In the past, teeth that had undergone root canal treatment often became dark in color because coloring medications or fillings were used. Today, such substances are no longer used. Nevertheless, the color of a root canal-treated tooth can later become somewhat gray, for example. However, discoloration can only occur in natural teeth; there is no such risk with artificial crowns. Discoloration can often be reversed by whitening inlays in the tooth (bleaching).
